Kindergarten Registration
If your child will be 5 years old on or before December 1, 2010, they may enroll in Kindergarten for the 2010-11 school year.
To attend St. Johns Public Schools, the State of Michigan requires the following information be on file before your child may start school:
· Child’s certified birth certificate – this will have the raised seal on it and we will make a copy of it AFTER we verify we have seen the original
· Copy of the child’s most recent immunization records –requirements for a child to enter school have changed, please check with your doctor’s office or local health department for updates
· A physical within the last year – if your child was in pre-school and the physical is less than one year old, you may use it
· A vision test within the last year
· Proof of residency verification – see white residency form for options
AND the following forms will need to be filled out entirely to enable the District to get your child enrolled.
· Student Registration– front and back
· Residency Verification and Internet Access Permission
· Transportation Request Form – if transportation is needed
NOTE: Forms are available at the bottom of the page under Links and E-Mails. You will only need to print the above mentioned forms from the list for kindergarten registration. If you are unable to print the forms, please feel free to contact our offices. Please fill out and return the above forms to the District Office, 501 W. Sickels St. St. Johns, MI 48879. You will be notified the second full week of August about kindergarten placement for your child. This notification will include the school they will attend and a.m. or p.m. placement.

Non-Resident and Schools of Choice Students
Non-Resident and Schools of Choice forms are available by clicking on the link below. Non Resident and Schools of Choice students will receive their kindergarten assignments during the 3rd week of August, after all other kindergarten students have been assigned. This assignment will only be good for one year. When they move on to the next grades, you will need to apply again for every year you chose to have your child attend of school out of their home area. |
