Physical

It is a requirement of St. Johns Public Schools that every student have state required immunizations, and that we have a recent copy of the student's official immunization record at the school building.

You may obtain a copy of your child's immunization record from the Mid-Michigan District Health Department, located at 1307 E. Townsend Rd, Saint Johns, MI 48879.

Although it is no longer a requirement that students enrolling in St. Johns Public Schools for the first time have a physical, "The Revised School Code (Act 451 of 1976, section 380.1177) requires that parents of children entering kindergarten present a statement to school officials confirming that they have received the Michigan Department of Community Health Vision Screening OR a statement, signed by a licensed eye care practitioner (optometrist or ophthalmologist) or medical/osteopathic physician, indicating that a child's eyes have been examined at least once after age three and before initial school entry, may also be presented." Although the school provides hearing and vision screenings at kindergarten roundup, it is the responsibility of the parent to get the child's vision screened prior to the first day of school.

Please check the Mid Michigan District Health Department's website at www.mmdhd.org/ for information on Hearing and Vision Screening Clinics.
